  • A comparison of healthcare utilisation, costs and the provision of low-value care in patients with and without evidence of cognitive impairment in the last year of life.

A comparison of healthcare utilisation, costs and the provision of low-value care in patients with and without evidence of cognitive impairment in the last year of life.

    Partner

Dr Brett Doble, Lien Centre for Palliative Care, Duke-NUS Medical School, Singapore

As the population ages, the number of patients living with cognitive impairment will increase and as the conditions progress, patients will eventually require end-of-life care.

Providing end-of-life care for patients with evidence of cognitive impairment poses unique challenges which include predisposition to infections, poor compliance to treatment and presence of co-morbidities. These factors could potentially result in significant healthcare costs in end-of-life care for these groups of patients.

This project aims to look at healthcare utilisation, associated costs, the appropriateness of care and any differences in these outcomes between patients with and without evidence of cognitive impairment in the last year of life. The team aims to quantify these outcomes or differences using MOH Mediclaims utilisation and cost data, and linking a subset of this data to the Singapore Chinese Health Study.

The findings will allow for a better understanding of end-of-life experiences, forecasting of future costs/utilisation, appropriate setting of subsidies and identification of opportunities to improve end-of-life care for patients with evidence of cognitive impairment.

  • An exploratory study on challenging interactions with patients and families in palliative care setting – its impact on healthcare providers and how to manage them

An exploratory study on challenging interactions with patients and families in palliative care setting – its impact on healthcare providers and how to manage them

    Partner

Dr Lee Chung Seng, St Andrew’s Community Hospital, Singapore

Although palliative medicine prides itself in managing complex communications in emotionally-laden circumstances, little is written about management or coping of these interactions by palliative care providers.

It is important to address interactions and their effect on healthcare providers, so as to encourage staff retention and prevent burnout. Challenging interactions could leave both the doctor and the patient feeling frustrated and dissatisfied, and can decrease the trust in the doctor-patient relationship. Understanding and addressing interactions can promote wellness and ensure good quality care can be provided to patients and families in this important field of medicine.

This study aims to understand what constitutes challenging interactions in a palliative setting from the perspective of the healthcare providers. It also aims to explore the impact of such interactions and the subsequent self-coping mechanisms used to manage emotions resulting from these interactions. The study will also aim to develop a series of practical responses a healthcare provider can adopt when dealing with these circumstances.

  • Impact of high-pressure processing on quality, nutritional and microbial safety properties of vegetable juices made from local produce to minimise food waste

Impact of high-pressure processing on quality, nutritional and microbial safety properties of vegetable juices made from local produce to minimise food waste

    Partner

Prof Zhou Weibiao, Department of Chemistry, National University of Singapore

Juicing is a potential solution to the problem of food wastage globally. Local farms produce vegetables from the Brassica family that are consumed frequently in Asia, and which are also potentially high in bioactive compounds. However, the impact of juicing on these particular compounds is not fully understood. Furthermore, sterilisation and pasteurisation of vegetable products are very challenging and vegetable products may also be contaminated with bacterial food-borne pathogens, which can lead to foodborne disease outbreaks. Heat treatments typically used to extend shelf life tend to destroy heat-sensitive nutrients and cause undesirable quality changes. High pressure processing (HPP), which is an innovative nonthermal pasteurisation process, is an attractive alternative.

This project aims to characterise the nutritional profile of juices produced from several locally-produced vegetables and to study the effects of high-pressure processing (HPP). As an innovative non-thermal processing technique, HPP is potentially more beneficial than the widely-practised thermal processing techniques, in retaining the nutritional quality of the juices and inactivating spoilage and pathogenic food-borne bacteria commonly implicated in foodborne outbreaks.

Findings from this project will be critical to enabling Singapore farms to convert excessive raw produce into high value-added food products of superb nutritional quality, by adopting cutting-edge technologies, while ensuring the safety of such food products.

  • Promoting Effective Biliteracy in Early Childhood: A Systematic Screening and Training Programme for Balanced Bilingual Development

Promoting Effective Biliteracy in Early Childhood: A Systematic Screening and Training Programme for Balanced Bilingual Development

    Partner

Dr Wu Chiao-Yi, Centre for Research and Development in Learning (CRADLE), Nanyang Technological University

A comprehensive understanding of bilinguals’ neural development (e.g. structure, function), cognitive capacities (e.g. linguistic abilities, working memory), and environmental factors (e.g. socioeconomic status, language exposure) is necessary to capture the various factors that might influence language and reading development. Existing literature demonstrates that phonological awareness (recognising sounds in spoken language) is one of the critical abilities for reading, and that training this awareness improves reading proficiency. Past studies however have focused on monolinguals and predominantly been based on alphabetic languages like English. It is therefore unclear if phonological awareness plays an equally important role for bilingual development when individuals learn both an alphabetic language and a non-alphabetic language like Chinese.

The project aims to develop an early screening and training programme to promote balanced bilingual development and biliteracy attainment in early childhood. The team will identify early neural, cognitive, and environmental predictors in pre-school years for later bilingual and biliteracy development. Additionally, the neurocognitive effects of different training programmes on enhancing biliteracy development will be evaluated. The knowledge gained will broaden understanding of the neurocognitive development of language and reading networks in young bilinguals. The findings will have significant implications for early screening and effective training for promoting balanced bilingual and biliteracy development.

This multi-year programme will introduce researchers to new palliative care approaches; increase capabilities of researchers in new approaches through sustained consultation with subject matter experts; enable better programmes and interventions, as new approaches adopted in the researchers’ projects can have a trickle down effect on research in this area; impact beneficiaries directly, including researchers trained under this programme.

The programme comprises two parts: Part 1: A workshop to introduce researchers to a new "realist" approach, which is especially useful for maneuvering complex interventions.
Part 2: Sustained mentorship for selected researchers to conceptualise projects and/or receive guidance during their ongoing research.

A/Prof Goh Wooi Boon, School of Computer Science and Engineering, Nanyang Technological University

A/Prof Goh Wooi Boon observed that most children seem to be more engaged and energetic when they are moving around and playing with their friends, and wondered if appropriate technologies could be developed to allow teachers to conduct lessons in a similar manner, with the goal of improving the engagement levels and motivation of students.

In a previous grant awarded in 2015, A/Prof Goh and his team conducted research which led to the development of i-Tiles, a tool that encourages play, movement and collaboration in learning, accompanied with lesson designs aimed at improving learning outcomes, sustaining engagement and increasing social interaction among children with special needs. Based on qualitative evidence from field trials conducted in the pilot centre, MINDS Fernvale Garden School, the i-Tiles pedagogy increased students' engagement and motivation to learn compared to regular classes.

This booster grant project seeks to continue to refine the I-Tiles system, and see to the expanded deployment and scale up of the pedagogy among more classes within MINDS Fernvale Garden, as well as to more MINDS affiliated schools.

Healthy speech sound development in early childhood is crucial for effective language acquisition. If not addressed early and effectively, speech sound disorders (SSD) negatively impact a child’s learning, literacy and socialisation. In Singapore, factors influencing speech sound development in young children are still not clearly understood, thereby limiting suitable smart strategies available for early detection and appropriate remediation. In this project, the team proposes to develop innovative speech-tracking Apps, aimed at developing a children’s speech database in Singapore, as well as to detect and classify (automatically) common speech sound errors to identify causal factors influencing SSD in Singapore children.

Consequently, the team will design appropriate interactive game based speech-training Apps to support traditional speech therapy and quantify the efficacy of such “smart” interventions. The research would contribute to the establishment of the first database of normal and disordered speech sounds in Singapore children, which could be relevant for other research, development of new technology (screening and speech training App) and enhancement of strategies by language/speech therapisst to address underlying factors in SSD.

Urban recirculation aquaculture systems for long-term culture of high-value crustacean species

Dr Mark Richards, School of Chemical and Life Sciences, Nanyang Polytechnic

Mud crabs (Scylla serrata and related species) are considered one of the most valuable seafood in Southeast Asia due to their fine taste, high quality meat, and good nutritional value. The demand for mud crabs has led to a drastic depletion of natural stock as well as unreliable and unsustainable growth of the mud crab industry.

In response to these challenges, crab farmers routinely utilise individual enclosures (so-called ‘crab condominiums’) for mud crab fattening and grow-out. Crabs kept in these recirculation aquaculture system (RAS) enclosures do not grow and moult as quickly or frequently as compared to those cultured in dug-out ponds. This is indicative of severe shortcomings with existing crab condominium design.

The project aims to develop and demonstrate proof-of-concept for a superior RAS system design for the long term culture of mud crabs and other high-value crustacean species.
